The Support Coordination Program provides intensive case management with treatment and community support strategies to address the challenges of self-care, productivity, and leisure.
Our multidisciplinary team includes support coordinators, a co-occurring addiction specialist, and a peer specialist. It collaborates with the person, family, and health and service networks to create their recovery plan and live in their chosen community.
